HI AND THANK YOU FOR BECOMING A FUR NANNY CLIENT.  WE WOULD LIKE TO TAKE A MOMENT TO ASSURE YOU THAT WE WILL DO EVERYTHING IN OUR POWER TO MAKE THIS EXPERIENCE AS EASY AND NON-STRESSFUL AS POSSIBLE.  WE HAVE SOME INITIAL PAPERWORK FOR YOU TO FILL OUT,  BUT THIS ONLY HAS TO BE DONE ONCE.  WE WILL RETAIN THE INFORMATION IN OUR FILES AND E-MAIL IT TO YOU ON YOUR NEXT TRIP SO THAT YOU CAN VERIFY AND MAKE ANY CHANGES. 
PLEASE SELECT THE COLUMN THAT PERTAINS TO YOU AND FILL OUT THE  APPROPRIATE PAPERWORK. 

IN HOME CLIENTS

1. Please read and print a copy of our policies document.  If you have any questions please contact us before continuing.  You will be asked to sign stating you have read these policies.
2. Please open the Client Information Document below.  Print the document and fill it in by hand and we will pick it up at the initial consultation.
3. Please open the Pet Information Sheet and complete one per pet or herd. 
4. Repeat the process with the Emergency Care Authorization form (only one form is required if the instructions apply to all pets and all pets use the same veterinarian) Please DO NOT forget to select a hurricane / natural disaster option 
5. Then complete the visit information sheet, if you are unsure about the number of visits  and length of you want please call us and we can help you decide
